How to Complete the Live Scan Application
Download the Request for Live Scan Service form. Make sure your applicants or volunteers include your project’s number as the OCA # on the Request for Live Scan Service form (middle section of the Live Scan application). This helps Human Resources to be able to notify you immediately when the results are received.
Submit a fax copy of the form to Community Partners once the applicant has been to the Live Scan agency of their choice. The fax copy gives Community Partners notification that a Live Scan service has been requested.
FAQs about the Live Scan Process
- What is Live Scan?
Live Scan is a fingerprinting service. We use this service for all Projects whose representatives, i.e. Employees, Volunteers, and Independent Contractors of projects who have direct contact with children, the elderly, disabled individuals, or any other vulnerable population.
- Is there a cost?
Yes. There is a rolling fee that varies from location to location. This rolling fee covers only the operator’s cost for rolling the fingerprint images. There are additional processing fees that are required for the State (DOJ) and Federal (FBI) level criminal record checks. Other fees may also be required (i.e. license fees).

- I have already been fingerprinted with another organization, or in another state. Do I need to do it again?
Yes. A person will need to be Live Scanned through Community Partners’ mail code number in order for Community Partners to receive the most current background check information regardless of previous live scans performed. Agencies are not allowed to share criminal record history information, including responses that no criminal record history exists.
